John Thomas Sr., 74, suffering from end stage black lung disease lies in bed in the family house close to death. His black lung benefits were taken away from him by the coal company. He is taken care of by his wife, son and daughter.
He worked in the cleaning plant for 22 years where the coal dust was said to be so bad you couldn't see your hand in front of your face. He died the day after this photograph was taken.
